We are able to offer, in conjunction with West Cheshire CCG, a wide range of community and practice-based services to our patients and these comprise of:

Community Care Team

We have a Community Care Team that are based at the Fountains Health in Chester City Centre. The Community care team is made up of District Nurses, Occupational Therapists, Physiotherapists, Community Matrons, and a Social Worker. District Nurses

 The District Nursing team provide home nursing care to patients who are unable to come to the surgery through ill health. Arrangements can be made by the surgery for a district nurse visit. The District Nursing team can be contacted on 01244 398013.

Health Visitor

 Our health visitor can give advice and help on health care, particularly for expectant mothers, babies, children, the elderly and disabled.

Counsellor

The Counsellor is specially trained to help those patients and their relatives who are under stress. Referral when necessary is made by your GP.

Midwife and Maternity Medical Services

The community midwife shares your antenatal care with the doctor. The midwives can be contacted on 01244 365166.

Patient Transport Service
