If you live within our practice area and wish to register as a patient please contact the surgery and they will explain the process.
If necessary, an appointment will be made for your new patient health check, which will be carried out by a member of our nursing team.
You will be registered with one of the partners although patients are free to consult with a GP of their choice.
We have a defined practice area. Please look at our practice map and enter your address to see if you live within our boundary. The green area is our outer boundary. This area allows existing patients who move from the yellow area to the green area to remain registered with the practice.
We require the following 4 documents
1. GMS1 Form
2. New Patient questionnaire
3. Photo ID
4. Proof of address
